## Description

Setmelanotide — called RM-493 during development — is an eight-residue peptide closed into a ring that activates the melanocortin-4 receptor (MC4R). It belongs to the same family of melanocortin analogues as melanotan-II, but was redesigned to prefer MC4R far more strongly than its close relatives MC1R and MC3R, an aim that runs through almost every published account of its pharmacology.

MC4R is a hub of the leptin–POMC circuit in the hypothalamus, which turns signals about the body's energy stores into central control over eating and energy use. Genetic defects further up this chain — in POMC, PCSK1 or the leptin receptor — leave MC4R working but under-stimulated, and that is precisely the situation researchers investigate with a selective MC4R agonist.
