The four compounds usually lumped together as "next-generation GLP-1s" are not four takes on the same idea — each stands for a distinct strategic wager: bolt on amylin (amycretin), refine the GLP-1 backbone itself (ecnoglutide), drop peptides altogether in favour of an oral small molecule (orforglipron), or bolt on glucagon (survodutide). When you choose a reference standard, the receptor target and the class of molecule count for much more than the headline percentage linked to each name.

Amycretin: a single chain for two hormone systems

Conceptually, amycretin is the most intriguing of the group because it is a single molecule. Instead of formulating a GLP-1 analog together with a separate amylin analog, its sequence is designed so that one chain activates both the GLP-1 receptor and the amylin/calcitonin receptor complexes. Early-phase results shown during 2025 described large mean reductions in body weight at roughly 36 weeks in the highest-exposure study arms, subject to the familiar early-phase reservations: few participants, short follow-up, and unresolved tolerability questions at the upper end.

In a research programme, ecnoglutide works best as a comparator that isolates a single variable. Measured against semaglutide in the same receptor assay, any differences can be traced to backbone and acylation chemistry rather than to additional receptor arms. Orforglipron: the non-peptide in the group

Orforglipron is the odd one out and calls for a different way of thinking. That is precisely the purpose of the molecule. If GLP-1 receptor pharmacology can be delivered orally by a small molecule that is easy to manufacture, the economics of supplying the class shift. Activity at the glucagon receptor is what survodutide has in common with mazdutide and the triple agonist retatrutide, so the three are typically investigated as a group.
